The Efficacy of Negative-pressure Wound Dressings in Necrotizing Fasciitis of Lower Extremities

Abstract
Necrotizing fasciitis is all kinds of disease of fulminating septic process that involves not only the full layers of the skin but all underlying structures, including muscular structures. The negative pressure dressing is widely used to induce and promote the wound healing process. The authors applied the negative pressure dressing to necrotizing fasciitis open wound in lower extremities to test out clinical impression. A retrospective chart reviews of 4 patients suffering from necrotizing fasciitis over a 3-year period was performed. All the patients was intensively treated in acute aggravating period by careful monitoring and coverage of broad antibiotics usage. After the stabilization of disease process, the negative pressure wound dressing was applied to prepare the wound bed for coverage. The efficacy was evaluated by comparing with the conventional wet dressing technique of wound care. Negative pressure wound dressing greatly reduce the wound size (36% vs. 32.9%) and shorten the period to coverage operation (29.8 days vs. 33 days), to compared with conventional wet gauze dressing. The negative pressure wound dressing was found to have an efficacy in managing the necrotizing fasciitis wounds. The negative pressure dressing should be considered as a good option in patients with necrotizing fasciitis.
